Check Your Eligibility

You’re eligible for benefits starting on the first day of the month following your date of hire if you are a full-time teammate working 30 hours or more per week (20 hours or more for VRS physicians), or immediately if they are hired on the first day of the month. We also cover your spouse, dependent children under the age of 26, and tax dependents over age 26 who are disabled and dependent on you for support on our medical, dental, and vision plans.

Making Changes to Your Benefits

Once you choose your benefit options, you cannot make changes or additions until the next annual Open Enrollment unless you have a Qualifying Life Event (QLE). Teammates have 30 days from the date of a QLE to add, change, or terminate dependent coverage during a non-open enrollment period.

If you do not add, change, or terminate elections during the 30 days, you will have to wait until the next plan year’s open enrollment or experience another QLE.

  • Marriage
  • Divorce or annulment
  • Legal separation
  • Birth of a dependent
  • Adoption/placement for adoption
  • Gain of other coverage
  • Loss of other coverage

Enrolling Your Family Members

Spouses (same or different sex) of a current vRad teammate and their children under the age of 26 are eligible for dependent coverage.

Required Dependent Verification

When enrolling a dependent for the first time, you will be required to submit proof that your spouse or child qualifies as a dependent. Here is a list of the documentation you may need to enroll your family members:

  • Legally married spouse: copy of marriage certificate (not license) along with a document showing both names with their common address, dated within 60 days*.
  • Child up to age 26: copy of child’s birth certificate (hospital or state/county issued) indicating the name of the parents*.
  • Stepchild up to age 26: Copy of child’s birth certificate showing relationship and documentation of teammate’s spouse* (as required above).
  • Legally adopted or foster-placed child up to age 26: copy of legal adoption or placement documents from applicable court of government agency (if the adoption or placement documents do not indicate birth date, provide a copy of the birth certificate).
  • Child (up to age 26) for whom the teammate has legal custody: copy of court decree granting custody (if the custody documents do not indicate birth date, provide a copy of the birth certificate).

*A receipt for a request of the certificate can also be provided, however, a copy of the certificate must be provided once received.
