Open Enrollment Reminders

Remember: The choices you make during Open Enrollment will remain in effect for the entire 2026 plan year, and you will not have another chance to make changes until the next Open Enrollment (unless you experience a Qualifying Life Event).

Your Open Enrollment Checklist

To Make Sure You Stay on Track During Open Enrollment, Here’s a Handy Checklist:

  • Read the details about Open Enrollment on this website and explore all the benefits available to you through Radiology Partners.
  • Review your current benefits, personal, and dependent information in UKG.
  • Consider enrolling in voluntary benefits.
  • Review your life insurance and financial account beneficiaries. While your beneficiary can be updated at any time, this is a good opportunity to make sure your designations are up to date.
  • Are you enrolling a dependent for the first time? You will need to verify dependent eligibility by submitting appropriate documentation or coverage will not be granted. See a list of acceptable documents to the right.
  • Review your dependent information and confirm a Social Security number has been provided for each dependent enrolled under your medical coverage. This is an IRS requirement and is used for year-end reporting purposes.
  • Once you’re ready, complete your enrollment in UKG through Okta.

Required Dependent Verification

When enrolling a dependent for the first time, you will be required to submit proof that your spouse/domestic partner or child qualifies as a dependent. Here is a list of the documentation you may need to enroll your family members:

  • Teammate’s legally married spouse: copy of marriage certificate (not license) along with a document showing both names with their common address, dated within 60 days*
  • Teammate’s domestic partner: copy of Domestic Partner Certification or Affidavit along with a document showing both names with their common address, dated within 60 days*
  • Teammate’s child up to age 26: copy of child’s birth certificate (hospital or state/county issued) indicating the name of the parents*
  • Teammate’s stepchild up to age 26: copy of child’s birth certificate showing relationship to teammate’s spouse/domestic partner (as required above) and documentation for teammate’s spouse/domestic partner* (as required above)
  • Child (up to 26) who is legally adopted, or placed for adoption with the teammate: copy of legal adoption or placement documents from applicable court or government agency (if the adoption or placement documents do not indicate birth date, provide a copy of the birth certificate)
  • Child (up to age 26) for whom the teammate has legal custody: copy of court decree granting custody (if the custody documents do not indicate birth date, provide a copy of the birth certificate)

*A receipt for a request of the certificate can also be provided, however, a copy of the certificate must be provided once received.
